Mobile Massage Checklist

Please ensure that:

• You have chosen the location for your massage prior to my arrival and that there is a minimum of 10 feet by 10 feet of level space cleared for me to set up in. If you need assistance with setting up your space, inform me ahead of time.

• You inform me in advance if I will be using stairs to access the space. 

• I have a clear path to walk through to access the space

• Any pets -though a welcome presence- are clear from my path as I am setting up and taking down my equipment. If pets are at risk of damaging my equipment please ensure they are separated from the massage session and my equipment. 

• You are prepared to receive your massage shortly after I set up, such as making sure the temperature of the massage space is appropriate for you, or that pets or children are okay for the duration of your massage.  

• You inform me in advance of any injuries or medical conditions that affect your mobility. 

• You inform me in advance if you have a medical assistant who will be available or present during the session.

Mobile Cancellation Policy, Late Policy, and Fee Schedule

• Your session time officially starts as you lay on the massage table. However any significant time delays in starting your session due to unpreparedness in the space may be detracted from the time allocated towards your massage. 

• Addresses over 7 miles from the address 1188 35th Ave, Sacramento, 95822 will incur a charge of $1 per a mile travelled

• Any disruptions during your session that result in a need to stop the massage will detract from the length of your session.

• Where applicable, provide 48 hours of notice when canceling your session. You may be charged anywhere from 50% to 100% of the cost of your session if you fail to do so. You can cancel or reschedule either by calling or texting (916) 426 8175, or by using the contact form on my website’s home page at yourbodykneadsmassage.com. If possible when communicating over phone, I prefer texts as I can respond to them faster.

• Repeated cancellations and rescheduling may incur a fee up to 50%-100% of the cost of your session. 

  
  

• Depending on the space I take a maximum of 15 minutes to set up my equipment and 15 minutes to pack it away. This is not counted as part of your massage time, however there is a charge of additional massage time if there is a hindrance in the space that keeps me from setting up or packing away my equipment in a timely manner. 

• If there is unexpected inclement weather the day of your session that affects my travel time I will be sure to inform you if I am going to be delayed as soon as I am able to. The amount of time added to my travel during inclement weather may incur extra travel fees which I will estimate upon my arrival. However if I foresee the additional travel costs may exceed $50, I am open to rescheduling without any additional charge. 

• No call/no shows will be charged the cost of your session. Prepaid sessions and gift certificates will become void if you do not show up to your scheduled session, and will not be refunded.

• Fees can be deducted from prepayment, this includes gift certificates. Any fees must be paid in full before booking another session. 

• Sessions are non-refundable

Mobile Massage Bodywork Agreement

In the following statements all references to 'I,' 'my,' 'me,' 'mine,' or similar personal pronouns are to be interpreted as referring to you, the client completing this form.

• I (the client) acknowledge that I am seeking massage therapy and that this service involves instances of the therapist touching my body. There may be times that I will need to be unclothed in order to receive certain modalities of massage. I understand that I may obtain information regarding any massage performed, from my therapist.

• If I experience any pain or discomfort during the session I will immediately inform my therapist so that massage or surroundings can be adjusted to my level of comfort.

• I understand that massage is not a substitute for medical care. Information exchanged during any massage session is intended to help me become more familiar with my body and is to be used at my own discretion.

• I have filled out the Health History form to the best of my knowledge, and notified my therapist of all known medical conditions and injuries. I agree to inform my therapist of any changes in my health in future sessions.

• In any instance during the session, if the therapist is not comfortable with moving forward with the massage the session will be ended and session costs will be adjusted accordingly. 

  
  

• In the event that equipment becomes damaged due to circumstances within my control (such as unsupervised pets or children), I agree to cover the reasonable repair or replacement costs of the damaged equipment.
