Browser-Friendly Body Work Agreement

• I acknowledge that I am seeking massage therapy and that this service involves instances of the therapist touching my body. There may be times that I will need to be unclothed in order to receive certain modalities of massage. I understand that I may obtain information regarding any massage performed from my therapist.

  
  

• If I experience any pain or discomfort during the session I will immediately inform my therapist so that massage or surroundings can be adjusted to my level of comfort.

  
  

• I understand that massage is not a substitute for medical care. Information exchanged during any massage session is intended to help me become more familiar with my body and is to be used at my own discretion.

  
  

• I have filled out the Health History form to the best of my knowledge and notified my therapist of all known medical conditions and injuries. I agree to inform my therapist of any changes in my health, in future sessions.

  
  

• In any instance during the session, if the therapist is not comfortable with moving forward with the massage, the session will be ended and session costs will be adjusted accordingly.

  
  

• I have read the cancellation/late policy.
